Rola CAMHS w EHCP - na etapie, gdy plan już jest.

W pewnej grupie na FB rozgorzała dyskusja nt CAMHS i możliwości uzyskania pomocy (sensownej najchętniej). Osobiście jestem tą, która mówi "od CAMHSu zachowaj nas, Panie!" i mam po temu mnóstwo powodów - od zawodowej wiedzy, poprzez doświadczenia klientów, po własne rodzicielskie i to powtórzone dwukrotnie. Niemniej jest to statutory service i gdy nie ma nic innego, to i CAMHS trzeba uczłowieczyć. W ramach "uczłowieczania" proponuję zapoznanie się z 2 przykładami tego, jak pomoc CAMHS skutecznie ujęto w planie EHC pewnego młodego człowieka.

PRZYKŁAD 1 (farmakoterapia i psychoedukacja nt. zaburzenia psychicznego)

Część E planu:
By [DATE], [CHILD] will independently manage their condition and treatment of [ROZPOZNANIE PSYCHIATRYCZNE].
Small steps:

1. [CHILD] will understand their diagnosis of [ROZPOZNANIE PSYCHIATRYCZNE] and its impact.
2. [CHILD] will know when, how and who to communicate their mental health concerns.
3. [CHILD] will be able to effectively communicate their mental health concerns to the identified professionals.
4. [CHILD] will responsibly and consistently manage their medication (knowing the dose, taking it as prescribed, ordering repeated prescriptions).

Część F planu:

1. A Family-Focused Intervention designed for adolescents with [ROZPOZNANIE PSYCHIATRYCZNE] will be delivered by CAMHS care coordinator to provide psychoeducation on [ROZPOZNANIE PSYCHIATRYCZNE] to [CHILD] and their family. - Who: CAMHS care coordinator with [CHILD] and their family. - How often: once a week. - How long for: total of 26 sessions (60 minutes each) as per FFT manual.
2. A relapse prevention/ management plan will be developed in liaison with CAMHS care coordinator to manage the [CHILD]'s diagnosis of [ROZPOZNANIE PSYCHIATRYCZNE]. - Who: CAMHS care coordinator with [CHILD] and their family. - How often: once a week. - How long for: total of at least 5 sessions (60 minutes).
3. A relapse prevention/ management plan developed in liaison with CAMHS care coordinator will be reviewed following each significant episode of [ROZPOZNANIE PSYCHIATRYCZNE]. - Who: CAMHS care coordinator. - How often: after each significant episode of [ROZPOZNANIE PSYCHIATRYCZNE]. - How long for: total of at least 1 session (60 minutes).
4. [CHILD] will see their consultant psychiatrist to monitor their condition and treatment of [ROZPOZNANIE PSYCHIATRYCZNE]. - Who: Consultant psychiatrist. - How often: every 2 months. - How long for: 1 session (30 minutes) at a time.
5. GP will provide repeated prescriptions of medications in liaison with consultant psychiatrist. - Who: GP. - How often: every 2 months. - How long for: 1 prescription for all medications to be written as per the shared care agreement at a time.

Część F planu:
[CHILD] will have one 1-hour session a week with an EMDR therapist through Child and Adolescent Mental Health Service (CAMHS) to explore different emotional states with a view to learn to recognise, understand and self-regulate them. - Who: Accredited EMDR therapist. - How often: once a week. - How long for: 1 session (50 minutes) at a time, a total of at least 26 sessions.

Nic lepszego od CAMHS nie udało mi się w życiu wydobyć. To, co poniżej, było naprawdę porządne i life-changing. Wszystko było w części F, więc w teorii, gdyby CAMHSowi się odwidziało, LA nadal musiałoby tego psychiatrę skądś wytrzasnąć.
